*Note: This information is used to screen patients for Medicaid/Family Health Plus and for programs that offer discounted medications. It may also be used to determine whether your income qualifies you for treatment at the WCCC. Otherwise, this information does not affect your healthcare or the other services available. If a family member or friend supports you, then please bring a copy of their proof of income.
The WCCC has a waiting list of several weeks for new appointments. To ensure that we operate at maximum efficiency, patients must notify the WCCC of cancellation at least 24 hours prior to their appointment.
