NY 15/15

Government program that provides supportive housing to people with mental health diagnosis or substance use disorder through a HRA referral

Help facilitate conversation between patient and HRA worker to help them apply. Help patient compile documents needed for HRA 2010e Supportive Housing application.

 

 

CUCS

Supportive and transitional housing given through a referral from HRA

Help facilitate conversation between patient and HRA worker to help them apply for housing through CUCS. Help patient send over mental health diagnosis to HRA for application.

Prevention Assistance and Temporary Housing (PATH)

All families with children seeking shelter in NYC must apply at the PATH intake center in the Bronx. Pregnant women and families with a pregnant woman may also apply for shelter at PATH.

Direct patient to PATH intake center and make sure they bring all required documents with them.
