Care for the Homeless: Primary Care Services

Primary care services program offers adult patients with physical exams, vaccinations, health screenings, chronic disease management and non-emergency on site care. The program also provides pediatric services such as nutrition counseling, immunizations and episodic illness care. The program also offers care coordination for LGBTQ patients

Offers psychiatric consultation, psychotherapy, substance use counseling and MAT
