Government program to provide monetary assistance to people in need

 

Help patient create my Social Security account on SSI website. Can then apply for benefits from website or call local SSI office for assistance.

HASA

Government assistance for individuals with HIV/AIDS, including housing

Call office or visit in person, submit application and supporting documents to join HASA, get assigned caseworker

ADAP

Government program that provides free medications for the treatment of HIV for patients that are uninsured or underinsured.

Help patient fill out ADAP application, including sending in proof of residency, income, and HIV status.

WIC (Women, Infants, and Children)

Government program that provides free food, nutrition counseling, and access to health services to low-income women and their children.

Find WIC local agency near patient, call that agency to make an appointment for patient. Check documents that patient should bring to appointment.

Access NY

NYC government website that asks screening questions and then gives you full list of all city/state/federal public benefits that you are eligible for.

Help patient go through eligibility screening survey on Access NY website. Then help patient get connected to benefit programs that they are eligible for.
