The Bridge Fund

Organization that is part of NYC’s Homeless Prevention Fund – gives interest-free loans and grants (around $2500) to people with rent arrears to help avoid eviction. Requires a referral.

Coalition for the Homeless

Organization that is part of NYC’s Homeless Prevention Fund – gives one-time grants of about $1000 to people who have had legal proceedings for them to evict brought against them, who have a signed court stipulation, and who demonstrate the ability to pay their rent going forward.

Call their Eviction Prevention Hotline Wednesday morning beginning at 9:30 am to schedule an appointment for the patient to discuss receiving a grant.

Community Service Society of New York

Organization that is part of NYC’s Homeless Prevention Fund – gives grants to those with mortgage or rent arrears to help avoid eviction. Also has separate program that gives grants to help with utility assistance.

CityFHEPS/ Homebase Program

NYC government program that provides financial assistance via specific organizations to people that are at risk of becoming homeless. Across the five boroughs, there are seven organizations that take part in the NYC Homebase program.

Look at borough maps on NYC Homebase website to see which organization covers their neighborhood. Contact that specific organization to get patient connected to HomeBase case manager.

BronxWorks – Homebase Program

Organization that offers NYC Homebase services in two locations in the Bronx, including short-term financial assistance for people facing housing insecurity.

Organization that offers NYC Homebase services in four locations in the Bronx, including short-term financial assistance for people facing housing insecurity.

Home Energy Assistance Program (HEAP)

New York government program that gives grants to help low-income households with home energy costs. HEAP has a variety of programs that can pay for: the cost of heating, utility arrears, heating equipment repair and replacement, air conditioner or fan. The money is given directly to the heating vendor or utility company.

Go through the list of HEAP programs to see which client needs.

 

Go through HEAP page on Access NY website. This will walk you through the steps to determine if the patient is eligible for HEAP benefits, what documents they need to apply, and how to apply. Note that HEAP applications are only open seasonally.
